Compliance Corner September 2025
Based on feedback from health centers, BPHC developed a new Operational Site Visit (OSV) Required Documents and File Naming Convention resource to help health centers better prepare for an OSV. This resource details which documents health centers are required to upload, what samples to select, and how to organize and label the documents for OSVs. It also includes an interactive checklist to track the documents health centers have uploaded.
The new resource is available on the Site Visit Resources webpage along with many other useful resources. If you have any questions about the new resource, reach out to the Federal Representative assigned to your OSV or use the BPHC Contact Form.

HRSA is adjusting the single audit threshold from $750,000 to $1 million in alignment with HHS’s Interim Final Rule (IFR), which took effect October 1, 2024. As a result, we are updating Chapter 15: Financial Management and Accounting Systems in the Health Center Program Compliance Manual (Compliance Manual) and the Financial Management and Accounting Systems section of the Site Visit Protocol (SVP) to reflect the new threshold.
The IFR provides HHS’s phased plan for adopting 2 Code of Federal Regulations (CFR) part 200, including the $1 million audit threshold at 2 CFR 200.301. Site visit teams are already using the adjusted SVP question in the Financial Management and Accounting Systems section during their reviews. We’ll provide more information as implementation continues.

To align with Administration priorities, HRSA removed data elements related to sexual orientation and gender identity (SOGI) in the Health Center Program Uniform Data System (UDS) reporting requirements. As such, HRSA does not ask for or require Health Center Program grant recipients to collect or submit data related to sexual orientation and gender identity (SOGI). Any statements (e.g., on health center websites, forms, or documents) that indicate the collection of SOGI data is a HRSA requirement are inaccurate and must be removed or modified. HRSA urges health centers to ensure removal of any such references, including by working with EHR vendors, subrecipients, contractors, and/or staff. Please refer to PAL-2025-03: Final Uniform Data System Changes for Calendar Year 2025 and the June 2025 Federal Register Notice for more information.
