Where do I find the HRSA Document List and Naming Convention

We have been preparing for our site visit but where do we find the HRSA Document Request list and naming convention?

The HRSA Consolidated Documents Checklist can be found on the HRSA website: https://bphc.hrsa.gov/compliance/site-visits/site-visit-protocol/site-visit-resources

This document request list provides all the documents that will be required for the Operational Site Visit (OSV). The Consolidated Documents Checklist with the Naming Convention (how to name the required documents) should be provided by the HRSA Federal Representative or the HRSA Consultant Team. Once your health center is contacted by the HRSA Consultant Team (the Team Lead), you may request the naming convention from them.

Proving Program Effectiveness Under Regulatory Scrutiny

Regulators are no longer satisfied with documentation alone; they want evidence that your compliance program actively prevents, detects, and corrects risk. Investigators expect to see how issues are identified early, investigated thoroughly, corrected effectively, and monitored over time. Boards demand measurable insight, and leadership needs confidence that exposure is managed before it becomes a liability. The standard has shifted from activity to impact.
