HRSA Progressive Action Conditions

As health centers are directed to have continuous compliance with the HRSA requirements, HRSA has now provided updated information regarding the Progressive Action conditions based on any elements that were out of compliance. In April 2020, HRSA determined that due to COVID-19 being a declared public health emergency, due dates for responses to Progressive Action items would be extended 90 days. The 90 days have passed and HRSA is NOW resuming the process. It is important that each health center (regardless of whether they are a FQHC or FQHC LAL), ensure that they do not have any outstanding conditions and work with their Project Officer to resolve those conditions. More information can be found on the HRSA COVID-19 FAQ located HERE. For more information on HRSA grant conditions and documents required to clear a condition, please refer to the Progressive Action Library located HERE.

Evidence-Based Strategies for Demonstrating Program Effectiveness

Regulators are no longer satisfied with documentation alone; they want evidence that your compliance program actively prevents, detects, and corrects risk. Investigators expect to see how issues are identified early, investigated thoroughly, corrected effectively, and monitored over time. Boards demand measurable insight, and leadership needs confidence that exposure is managed before it becomes a liability. The standard has shifted from activity to impact.
